Most services that we provide are covered by your medical insurance. Our fees are determined by insurance company guidelines and are comparable to those of other specialists in the area. To simplify your insurance process, we are more than happy to file your insurance forms for you.
It is important that you refer to your insurance policy regarding possible requirements such as referral from your primary care physician or a second opinion before surgery. We encourage you to become familiar with the terms and conditions or your policy, because ultimately, the responsibility for payment is the patient’s.
We will do everything we can to help you afford the treatment you need and want. A complete payment plan is designed with an appropriate payment schedule. Forms of payment accepted by the office are cash, check, or any major credit card.
If you are unable to keep an appointment, we ask that you kindly provide us with at least 24 hours notice. We ask for this advance notice so that we can offer this appointment to another patient. A fee may be charged if a patient does not show up for an appointment without sufficient notice.
